Major Depressive Disorder — Mental State Examination
Conduct a focused mental state examination on a patient presenting with low mood. Demonstrate systematic assessment of appearance, behaviour, speech, mood, affect, thought form and content.
Introduce yourself and obtain consent. Begin with open questions about how the patient is feeling. Systematically work through each MSE domain. Pay particular attention to affect — describe its range, congruence with mood, and reactivity.

You are James, 22, a university student. Over the past 3 weeks you've been hearing voices commenting on your actions. You believe your lectures are being recorded to monitor you. You're sleeping poorly. If asked directly, admit to the voices reluctantly.
Rapport and communication (0-3). Elicits hallucination history — type, frequency, content (0-3). Explores delusional beliefs systematically (0-3). Assesses risk to self and others (0-3). Formulates appropriate initial management plan (0-3).
